'Hospitals Face Pressure to Avert Readmissions'

Last month Medicare began levying penalties on hospitals it says have too many readmissions.  This pressure on hospitals is having a ripple affect on physicians in the community and home health care agencies such as VNA Community Health.  Our 'Well Right Now" program is in place to address the needs of individuals with chronic illnesses.
